Acupuncture Intake & Diagnostic Session

Please wear loose comfortable clothing. Your session is in a private treatment room; you are welcome to disrobe to your comfort level. If that means you would just like to roll up pant legs and sleeves, please dress accordingly. You receive a welcome email confirming the scheduling of your appointment. There are 2 links in the email. Please click on the links to download the Intake Consent document and a Health History form. Please print, complete and bring these with you to your intake appointment. If you have trouble downloading these documents, please let us know so that we may make other arrangements for you. We advise patients to have a light meal before their appointment. It is important that you are not running on empty during your session. We treat people with chemical sensitivities at our clinic; we ask you to please be mindful of that and refrain from wearing perfumes, colognes or heavily scented products on the days you visit the clinic.
